Yes you do. As a notary, my role is to verify the identity of the signer. Therefore, the name printed on your ID, is the name you must use when signing your documents.
I have a suffix in my name that is on my ID, but I never sign with it. Do I have to sign using my suffix?
Yes just because you don't use the suffix, does not mean that it is legally acceptable when signing documents. Your name must include your suffix when signing documents.
